Fees and Payments

Co-Payments and Balances due from Deductibles and Co-insurance amounts are the patient's responsibility and are due at the time of your appointment. An administrative fee will be accessed on your account if you fail to do so. For your convenience CIM does send out both ebills and paper statements. The email address on file will be used to send out the ebill. If you would like to opt out of ebills, please inform us at check-in or send us a notification under Patient Billing.

We accept payments in the form of Cash, Check, Debit or Credit cards. You can also pay your bill online at www.myproviderlink.com

Our apologies but we do not accept Medicaid as your Primary or your Secondary insurance and we are considered out-of-network and unable to see you as a patient.
